33rd Annual Ribbon of Life: Summer of Love

This represents and warrants that, on the date and time set forth above, the undersigned acknowledges that:                                                                                                                                                    

1. He/She is over the age of 18*; and *(In the case of children under the age of 18, a signature by a parent or guardian is required)                                                                                                                     

2. He/She assumes full responsibility for, and risk of, bodily injury, death, or property loss or damage whether due to the negligence or intentional acts of Golden Rainbow and/or Tropicana Las Vegas or otherwise; and                                                                                                             

3. He/She fully understands and assumes all risks involved in being provided access to areas in The Legends in Concert Theater or Havana Room at the Tropicana Las Vegas involving, but not limited to, stairs, pits, sets, equipment, rigging, stages and otherwise which may result in bodily injury or damage to property; and  

4. He/She hereby fully agrees to allow Golden Rainbow and The Tropicana Las Vegas to use his/her likeness and performance in any photographs, films, videos or recordings, including negatives and any prints or copies thereof, relating to Ribbon of Life 2019 in any form so desired by Golden Rainbow or Tropicana Las Vegas                                                                                         

5. The undersigned has read and voluntarily signs this RELEASE.                                                             

In consideration of the above and other good and valuable consideration, the receipt and suffice nary of which is hereby acknowledged, the undersigned does hereby agree to indemnity, defend and hold harmless and does release and forever discharge from all manner and actions, cause and causes of actions, suits, claims, controversies, damages and demands whatsoever, which the undersigned now has or which he / she or his / her parents or guardians, or his / her heirs, executors and administrators can, shall or may have against Golden Rainbow, its employees or Board Members and/or The Tropicana Las Vegas, its officers, directors and employees for or by any reason of any matter, cause or thing arising out of or in connection with his / her access to or use of the areas described above.
